A 44-year-old advocate came to me after struggling with knee and back pain for a long time. He had already undergone knee surgery, but instead of feeling better, his pain became worse. Walking, standing, climbing stairs, and even carrying out simple daily activities had become difficult. More than the pain, he was afraid that he might need another surgery.
Under the guidance of Col. (Dr.) Rana K. Chengappa, my first priority was to reduce his pain and help him regain confidence in the rehabilitation process. We carefully chose a treatment plan that would not aggravate his symptoms. We avoided lumbar spine extension exercises and did not perform joint mobilization, as these were not appropriate for his condition at that stage. Instead, we focused on gentle soft tissue release and streches to relax tight muscles and used spinal traction with a Mulligan belt to help reduce pressure on the lower back and tingling sensation over thigh and ease his leg symptoms. As his pain settled, we gradually introduced simple strengthening exercises for his hips, back, and legs to improve stability, balance, and walking. He was initially hesitant about dry needling, but after understanding the procedure, he agreed to try it and experienced significant pain relief.
The biggest relief came when further medical evaluation confirmed that surgery was not required. From that point onward, he remained dedicated to his rehabilitation, attending sessions regularly and performing his exercises consistently.
Today, he walks confidently, manages his professional responsibilities without difficulty, and enjoys his daily life without the severe pain that once limited him.
